It will print four pages that you must staple together and turn in to the Athletic Director as one stapled document.
All Four Pages must be completed and turned in stapled together to the Athletic Director, Mark Zalin.
Pages 1, 2 and 4 must be completed by the parent
Page 3 of the document (Medical Examination-Clearance Form) must be completed by a licensed physician.
Information about the Medical Examination-Clearance Form:
You can get your physical with your own doctor OR you can get a physical completed at the high school for $30 (cash only) on May 29, 30 and 3, 2013 between 2 pm and 5 pm in the small gym.
You MUST have your Physical Form with you to get your physical. NO FORM , NO PHYSICAL!
- Current Juniors 2-3pm
- Current Frosh and Sophomores 3-4pm
- Current 8th graders 4-5pm
There will be some flexibility but adherence to these times will help shorten wait time.
